Spring Sports

Registration must be completed through FinalForms.
 

• Parent/Guardian Action Required: Only a parent or guardian can register an athlete. Students may not register themselves but will need to complete the required student sign offs.
   
• New Accounts: If you haven't used the system before, you will need to create a new account.

Every student requires a new sports physical each school year.

• Validity: For the 2025-2026 season, physicals must be dated on or after June 1, 2025.

• Format: Pictures of physical forms are acceptable as long as all pages are included and the information is clearly readable.

All of the following steps must be finished BEFORE the first day of tryouts/practice:

1. Register your student on FinalForms.
2. Submit the Physical using one of these methods:
   • Upload (Preferred): Add directly to the student’s FinalForms profile.
   • Email: jameson.jablonski@nhsau57.org or scott.insinga@nhsau57.org.
   • Fax: 603-893-7084
   • In-Person: Drop off at the SHS Athletic Office or mail to SHS-Athletics.
